One visa application form duly filled and signed by the applicant.
Photo Specification
Two recent passport-size photographs with matt or semi-matt finish, 60%-80% face coverage, light background and without border (Size: 35mm x 45mm).
Please Note: Photograph should not be more than 3 months old, scanned/stapled and should not be used in any of the previous visas.
Approval Letter
Original Approval Letter from Ministry of Kuwait.
Police Clearance Certificate
Original Police Clearance Certificate issued by Passport Office and attested by the Ministry of External Affairs. The date of the P.C.C. (Police Clearance Certificate) must be applied after the date of the approval letter received from Kuwait.
Please Note: The clearance should be endorsed on the passport.
Documents of Spouse
a) Copy of first and last page of the passport and visa page.
b) Residence copy of Spouse.

a) Copy of Marriage Certificate.
Medical
Medical certificates to travel to Kuwait should be issued from panel of doctors authorized by the Embassy.
