Requirement Details
Passport Original passport with a validity of more than six months and minimum two blank pages (back to back) for visa stamp. Attach all your old passports (if any).
Cuba Visa Application Form One visa application form to be duly filled and signed by the applicant.
Photo Specification Two recent camera-clicked passport-sized colored photographs on a white background with 80% face cover on a matt or semi-matt finished paper (35mm x 45mm).
Please Note: Photograph should not be more than 3 months old, scanned/stapled and should not be used in any of the previous visas.
Covering Letter Original Covering Letter from the applicant on the company's letterhead stating his name, designation, passport number, purpose of the trip, duration of the trip & who will bear the expenses.
The letter should be addressed to:
The Visa Officer, Embassy of the Republic of Cuba, New Delhi.
Invitation Letter An Original invitation letter from Cuban Associate should be sent directly to the Embassy through fax and a copy should be sent to us.
Ticket Confirmed Return Air Ticket.

Note: Following details must be present on the ticket (for all sectors):
a) Booking Reference Number
b) Booking Status (Should be Confirmed)
c) eTicket Number
Financials a) Bank statement for last 6 months.
b) Foreign exchange endorsement / Copy of credit cards.
Yellow Fever / Health Regulation Important Note: Applicants are advised to check whether Yellow Fever Certificate is required or not, including vaccination requirements and recommendations for International Travelers.

Health regulation for passengers leaving India:
There is no health check requirement by Indian Government on passengers leaving India. However, persons leaving for a yellow fever infected area are advised in their own interest to get them vaccinated and to be in possession of valid yellow fever vaccination certificates before they leave the country.

Covering Letter Covering Letter from applicant on business letterhead mentioning name, designation, passport number, purpose and duration of visit in brief. The letter should be duly signed by authorized signatory with company stamp and addressed to:
The Visa Officer, Embassy of the Republic of Cuba, New Delhi.

Please Note: Covering Letter on Letterhead if applicant is Self Employed or on plain paper if Employed.
Financials a) Company's and personal ITR.
b) Six month's salary slip.
c) Company and Personal Bank statement for last six months.
Proof of Occupation Self Employed:
a) Company registration certificate / Certificate of Incorporation.
b) Brief company profile.
c) Articles of Memorandum (In case the applicant is MD/Director).
d) Proof of Proprietorship / Partnership (In case the applicant is Proprietor / Partner in a firm).
e) Import / Export license (if applicable).

Employed:
a) Proof of employment like salary slip or appointment letter / NOC from employer.

Student:
a) Leave sanction letter from the school.
Proof of Accommodation Hotel confirmation.
Ticket Confirmed Return Air Ticket.

Affidavit An affidavit (attested by a Notary Public) stating the Applicant's name, address, contact number, passport number, designation, purpose and duration of the visit. It should also mention that the applicant will not take up any sort of employment in Cuba during his stay duration and also depart from the country within the stipulated time period.
Personal Presence Personal presence of applicant is mandatory for submission of documents. However, UDAAN can facilitate the visa processing without personal presence.
