Passport: Original passport with validity of minimum six months and minimum two blank pages for visa stamp.
a) Copy of first and last pages of passport
b) Attach all your old passports (if any).
Bolivia Visa Application Form: Visa application form has to be filled online https://portalmre.rree.gob.bo/formvisas/ . Post filling the online form, printout of the application form duly signed (in blue ink) by the applicant has to be taken and should be submitted at the Embassy along with the other documents.
Following documents needs to be uploaded while filling up the online form.
a) Photograph
b) Itinerary
c) Passport copies
d) Yellow Fever Certificate
e) Documented Economic Solvency for last 6 months
f) Invitation letter from legal person with address in Bolivia / Book Accommodation
Photo Specification: Four recent passport size colored photographs with matt or semi matt finish, 60%- 80 % face coverage, with clear blue background and without border (4cmx5cm)
Please Note: Photograph should not be more than 3 months old, scanned /stapled and should not be used in any of the previous visas.
Covering Letter : Original Covering Letter from the applicant on company’s letter head stating his name, designation, passport number, purpose and duration of visit. The letter should be signed by authorized signatory with company stamp and addressed to – The Visa Officer, Embassy of Plurinational State of Bolivia, New Delhi.
Invitation Letter: Original invitation letter from Bolivia host/inviting company on company's letter head stating the purpose of travel and duration of stay and should be duly signed with signatory's name, designation, and should be addressed to - The Visa Officer, Embassy of Plurinational State of Bolivia, New Delhi.
Financials:
a) Last three months bank account statement.
Airline Reservation: Confirmed Round trip Ticket itinerary.
Medical: Original YFC (Yellow Fever Certificate) has to be submitted along with the visa application & applicant has to carry the same at the time of travel.
 Police Clearance Certificate: Original PCC (Police Clearance Certificate) issued from Passport office has to be submitted along with the visa application.(Mandatory)
