Scheme Visa Application Form Visa application form must be filled online https://visaonweb.diplomatie.be/ . Post filling the form, 2 printouts has to be taken and duly signed by the applicant on column 37 and 38.
Please Note Applicant mobile number and Email Id should be mentioned in visa form as Embassy / Consulate can contact the applicant at any point of time during visa processing.
Photo Specification Two recent passport size color photographs with matt or semi matt finish, 80% face coverage, white background and without border (Size: 35mm x 45mm).
Please Note – Photograph Photograph should not be more than 3 months old, scanned/stapled and should not be used in any of the previous visas.
Covering Letter Covering-letter from applicant on company’s letter head stating his name, designation, passport number, purpose and duration of visit along with his proof of occupation duly signed by authorized signatory and addressed to – The Consulate General of Belgium in Mumbai.
Invitation Letter Invitation Letter from Belgian host / inviting Company on its letter head stating the purpose of travel and duration of stay and should be duly signed with signatory’s name and designation.
Financials a) Personal ITR for last 3 years
b) Bank Statement for last six months
c) Six months salary slip
Proof Of Occupation a) Company Registration Certificate / Certificate of Incorporation
b) Brief Company Profile
c) Articles of Memorandum (Incase the applicant is MD/Director)
d) Proof of Proprietorship / Partnership (In case applicant is Proprietor / Partner in a firm)

Proof of Accommodation Hotel confirmation for the entire period of stay in Scheme area.

Please Note Hotel should match the ticket dates and maximum stay should be in Belgium.

Medical Copy of Medical travel Insurance for the stay duration amounting to 30,000 Euros. Insurance to be done by the Authorized Insurance Companies.
Medical – Please Note Travel insurance policy submitted along with the Belgium visa application must be signed by the insured person on the place reserved for it in the policy document. The insurance policy document will not be considered valid if it is not signed by the insured person and the visa will not be issued.

Passport Collection Process

Details
With immediate effect, Consulate has announced that representatives cannot collect the documents on behalf of an applicant from Belgium Visa Application Centers except in following conditions:

a) Immediate family member (that is Parents, Children, Spouse)
b) One member of a family group collecting the passports of the whole family.
c) A person collecting on behalf of Government officials if authorized on the Government letter head or department.

If any of the above comes for passport collection they need to carry the following documents:

a) Original Government ID card
b) Original ICR
c) Original authorization letter from applicant

However, applicants who do not wish to collect their documents in person and whose documents cannot be collected by one of the above, will get their passports back by courier at an additional cost of Rs. 300/- to the dispatch address mentioned in the Covering Letter which has to be submitted along with the visa application.
Please Note:

a) In case of a business applicant, if the applicant wishes to receive the passport to his/her office, then the same needs to be mentioned in the covering-letter.
b) For application withdrawal cases, the passport needs to be collected from VFS only.

New Procedure for Scheme Visa Applications

With immediate effect, Indian citizens travelling to Scheme countries for short-term (max 90 days in 180 days) will have to visit VFS visa application centre personally to provide biometric data (fingerprints and digital photograph). For subsequent applications within next 5 years the biometric data will be copied from the previous visa application included in the VIS.

Letter of Guarantee A letter of guarantee from shipping company.
Additional Documents a) Original and Copy of CDC (Continuous Discharge Certificate).

Invitation Letter A letter of guarantee (in the prescribed form “Annexed 3bis”/“Bijlage 3bis” - French - Flemish) by a guarantor permanently residing in Belgium or of Belgian nationality (in case he/she is NOT residing in Belgium). This document can be obtained from the guarantor’s municipality (or a Belgian mission abroad) where it has to be certified. It is valid for 6 months after its issue.
Financial Requirements of Guarantor Family visit (mother, father, brother, sister, in-laws): the guarantor must earn at least € 800 net/month + €150 for each of the guarantor's current dependents and/or + € 150 for each person invited.

Friends visit: the guarantor must earn at least € 1.000 net/month + € 150 for each of the guarantor's current dependents and/or + € 200 for each person invited.
Official Proof of Solvency a) Copy of the guarantor’s 3 most recent salary sheets / guarantor’s most recent tax sheet / other official proof of the guarantor’s solvency.
Remark: A letter from the bank or deposit account statement will not do.

b) A photocopy of the identity card or the unlimited residence permit of the guarantor.
Remark: A person with a limited residence permit cannot legally act as a guarantor.

c) An original and authenticated Certificate of family composition on the name of the guarantor, issued by his/her municipality in Belgium.

d) A photocopy of the Belgian identity card or residence permit of the family member you intend to visit in Belgium.

If the applicant is exempt from the work permit/training permit the relevant certificate should be provided.
Police Clearance Certificate A Police Clearance Certificate for the past 1 year. This document is valid only 6 months from the date of issue. In case you resided in a foreign country during the past 1 year, you should submit a Police Clearance Certificate issued by the relevant authorities of that foreign country.

Administrative Fee Applicant applying for Belgium D-Visa (long stay) has to pay an administrative fee apart from the normal visa fees. The administrative-fees has to be paid directly on the Home Affairs’ Belgian bank account, either from Belgium or from where the applicant is registered in EURO and all the bank/intermediate bank charges has to be borne by the person making the transfer.
Please Note Application won’t be submitted at Belgium Visa Application Center if the Fees is not paid.
